I-693A (vaccination supplement)

Hi, the civil surgeon gave me two originals of the I-693A, each placed in a sealed envelope: one addressed to the USCIS and one for me.

question: do i send the sealed envelope addressed to USCIS or should i just photocopy my own copy and include it in the AOS package? Then maybe during the interview i can bring the USCIS envelope. Just making sure it doesn't get lost or misplaced or something...

what did you guys do?

I sent the sealed one for the USCIS and kept the other one as a back up to photocopy if required.

Send USCIS the Sealed one addressed to them, and keep the other as backup for AOS interview.

The supplemental should be in a sealed envelope but CS should have made a copy for you.

You had a good doctor, you keep the envelope addressed for you and for your own records, and then you will include the envelope addressed to USCIS and mail in to USCIS in your AOS packet.

Lucky you, our Civil Surgeon wouldn't give us a copy so we were holding our breath that she had even completed it in our favor. I only had the receipt and a copy of the envelope as my own "proof". The Civil Surgeons are given very strict instructions on how to complete the forms and how they should be submitted. They are instructed to place the document in the sealed envelope....ours did, including signing the back seal and taping it as if we would try to get into it.

i was lucky to find this Civil Surgeon after calling about a dozen doctors who insisted on doing a full medical exam. You should have been given a copy of the I-693A, it was stated very clearly in the technical instructions for civil surgeons that they should give a copy to the patient. Our doctor really knows his job and charged us $70 for the TD shot and the form.
