Did you try the instructions to the form? It would be the Chicago lockbox address.

That's the thing I only downloaded the .pdf , no instruction was included.

Thanks again.

I do not think New Yorkers are supposed to send theirs to Chicago. I think it is supposed to be sent to Missouri. I received the wrong information on the phone and was told to send it to Vermont. My application did end up gettign forwarded to Missouri, but I know sometimes they are not so gracious. Find out from USCIS. I'd ask right at Federal Plaza is you can make it there. If you can;t do that, I would recommend sending it to the Missouri Office if I were you.

Aug 2002

Entered US on TN work permit

2003 through 2007

Renewed TN Annually

March 2007

Eloped in Central Park on vernal equinox, then honeymooned in Paris.

April 2007

Early April: Renewed TN on return from Paris

June 2007

Sent in AOS/I-130/EAD package

July 2007

July 14: DOA1 - delay in processing because CIS told me to send package to the wrong center.

Late July: Received Biometrics notice

Aug 2007:

Aug 2: Biometrics Appointment

Aug 8: Received RFE for missing medical forms which CIS told me originally not to send in!!

Aug 16: Medical Forms Received at CIS

Aug 24: Received e-notice that EAD card was ordered (No more TN! yay!)

Aug 30: Last day of work on TN. Laid off from job. :(

September 2007

September 5: Received Card! Just in time. Started part-time grad school today.

October 2007

Oct 2: Received Interview Notice: Whoo-hoooo!

November 27 2007

Had GC interview, which went smooth as silk. Received passport stamp, too.

Hi,

Are you just applying for I-485?

What about I-765 and I-131?Do you need employment authorization card or advince parole (traveling document)?

Anyway,what I sent with I-485 is:

1)copy of birth certificate follow by english translation!

2)copy of passport page with nonimmigrant visa

3)2 photos (i put in plastic bag, and attached to white paper)

4) I needed also I-693 (medical examinationa), but if you are K-1, and if you had medical examination within past year then you need to submit only vaccination supplement I-693A, you can download it from uscis.gov if you don't have one. What you have to do is find local surgeon in your area!!!

5)You also need to submit form G-325A (dowload it from uscis.gov it is biographic information sheet)

6)Submit a copy of your I-94, nonimmigrant arrival/departure record

7)You also need to submit I-864 (Affidavit of Support) follow by supporting documents

I just wanna remind you that you should apply at the same time for I-765 and I-131 with also supporting documents (if you didn't know this) because everything is going in same envelope!!!

If you need something just ask!

Alien number usually started with A letter in front and it is looks like A123456789 , i didn't have one since I was on another visa, but check you maybe have one on your I-94, or in passeport I am not sure, maybe some else can help about this!!

Good luck!!!
