Looking thru the returned forms and evidence. On I-485 part three at the end it asks "have you ever applied for permanent resident status in the US"

We did file, but got rejected. Since it was rejected, does it count? I think I should leave 'no' checked but I want to be sure.

Apllied is the same thing as filed!!

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

So check "yes"

They ask for something else then - I would assume a copy of the rejection notices would suffice, seeing as there is nothing else I could give them.

Right or wrong?

My thinking was that "yes" meant a person was here, filed, left, and filed again for whatever reason...

You don't need to give them anything.

Just put "yes" and fill in place, date and result.

USCIS will have it in their data base.

Wondered why you were denied the first time?

Same case. I'm still not sure why we go the rejections. Filed AOS, EAD, and AP at the same time. The only mistake I can find was on the AP. I posted about my confusion over the rejections and no one had any ideas what they wanted. Learned that I didn't add 2+2+2, so I only sent 2 passport pics - but that shouldn't cause a total rejection. The check was correct, if it was not they would have said the check was wrong. Made an infopass appointment for Wednesday.
