I have rang different doctors to see what they are charging to sign this I-693, all are over $100 just to sign the document, also i have to have the chicken pox injection which one doctor quoted $195 is this correct?? If anybody knows anyone around the seattle area that is cheaper and could provide a link or phone number it would be appriciated.

Definitley call around. Different civil surgeons charge different prices.

We were quoted (for the vaccination supplement) $50 (lowest, and the ones we chose) all the way up to $595.

Check with health departments and doctors that specialize in children's medical care for the varicella vaccine. The civil surgeon's fees are ridiculous for the chickenpox shot.

All I can tell you is that this whole thing is one giant rip-off.

I'm getting the same thing done tomorrow! I called up loads of places and most ranged from $100+ , one place I called were charging $200 with vaccinations at $75 . Luckily I found somewhere that will charge $25 for the form to be filled in and i'm getting the chickenpox vaccination for $18. It's an approved site in Minnesota. I would recomend calling all of the approved places up within reasonable driving distance and choose the cheapest.

I had the chicken pox done for $15 at the local health clinic.... then got the civil surgeon to add it to the paperwork....

Hello mate - Fingers crossed for the gunners in the semi finals.

I was stung by the vacc supp as well. They charged 285 dollars. To be honest it does vary dramtically - I have heard some people paying as low as 20 bucks.

The civil surgeon here charged $50 for the form and $85 for the chicken pox vaccine. I've read of a few VJers (can't remember who) who have driven 2hrs away to get a good price for this form.

What you were quoted seems a bit excessive alright

I am from the UK and have had my AOS approved, all I had to show on my vaccination supplement form was that I had had the MMR and TD jab, no chickenpox jab. The civil surgeon only charged me $25 to fill out the form.

I phoned this office and found them to be really nice and informative, the nurse said that I do not need the chicken pox injection because I had the pox as a child and she also asked my age. Also she asked me if I had all my other injections and to bring the documents (which I have with me) to the doctors tomorrow at 1:pm.

Hi there

I know this is a bit late janice but I got charged $125 for a full medical by my doctor. The jabs were done via a state health clinic Im not sure how that works but they have free clinics on certain days that anyone can attend it may only cost like 10 dollars . I got my sons done there for school... Cost 10 dollars or something.. Look up the local Health Dept in your area. They would for sure have these types of clinics.. Plus my doctor gave my daughter the jabs under the state program.. I guess I was lucky it only cost like 8 dollars for Hep B.. Schools are very informitive for info on shoots....
