Sorry to start another thread about vaccinations, but the others seemed to be going in different directions and I think my wife's worksheet seems particularly confusing.

She has had all her vaccinations in the past, but never had any documentation.

It seems like the civil surgeon in Madras checked multiple boxes which seem contradictory (although obviously the K1 visa and everything worked out ok.)

The actual worksheet with the boxes for each vaccination is completely blank;

Underneath there is no checkbox in 'vaccination history incomplete' but there is a check in its subheading 'applicant maybe eligible for blanket waiver because vaccinations not medically appropriate, as indicated above.' "K1" is written next to this, perhaps meaning that since it's a K1 visa, the vaccinations are not needed. Of course, that is not a medical reason.

Then, on the next line, there *is* a checkmark by "vaccine history complte for each vaccine, all requirements met (documented above.) Except there is no docmentation above.

Then on another page, there is no check by 'vaccination history complete'; there is a check below it by 'incomplete vaccine history, no waiver requested'; then there is no check by 'vaccine history incomplete, requesting waiver (indicate type below)", but there *is* a check below by 'blanket waiver' with K1 written below it.

So it seems like in this place it says a waiver is requested because it's K1 (even though 'no waiver requested') is checked.

Then on the worksheet it looks like it's marked as complete based on my wife's word, but without documentation; but also marked as a waiver for medical reasons (or just for the bureaucratic reason of being a K1 visa).

My main question is, will a civil surgeon just transfer info based on the 'vaccination history complete' without detailed documentation? seems unlikely.

Seems like the best thing might be to just draw some titers and things like that? Probably mmr, hib, dtp would be dismissed as age inappropriate; a given varicella history would take care of that; then we should draw a hep B titer - that would just leave pneumococcal and flu shots to get now, which would be appropriate enough. Maybe I should call a civil surgeon first and discuss a strategy like this.

my husband and i had the same problem....he is in El Salvador..

he also didnt have any documentation....

instead of paying more money when he went to the medical interview...we just had another doctor vacinate him again...it was terrible poor thing got like 6 shots in one day!

but they need do need proof from a doctor previouse vacination.....

My wife suggests just sending in a copy of the foreign medical form for the visa and just seeing what happens. that strikes me as a pretty bad idea.
