Send in a copy of your marriage certificate along with everything listed in the guides pusbrk referred you to. Affidavits are the easiest evidence at this time.

Check your documents againts the Example Forms link above. A mistake corrected now will save much time in the long run.

A check is good source of payment as you can see when your check clears and possibly get the receipt number from the back of your cancelled check.
