Is there any particular format to the I-134 employment verification letter we need from our employer? Can anyone provide an example format?

Also, do we need this from our direct manager/supervisor or can we get it from our HR Department?

HR department is fine.

For my now-husband's letter, we went with something like this:

"I'm writing to confirm Bee's employment here at Brown Parcel Services. He started with us in the beginning of 2001 as an Espresso Delivery Boy, and was promoted to a Mailroom Underling in the summer of 2003. He received the promotion for his current position of Head Honcho at the beginning of 2007. His position entails the duties of supervising Espresso Delivery Boys and training Mailroom Underlings.

His salary is three million seashells a month, which adds up to thirty-six million seashells in annual income."
