DS156 questions

I once had a green card, but since I was out of US for more than one year, my green card is no longer useable.

I then applied for SB-1 (Returning Resident Visa), but it was denied.

Here is a question.

1)Should I have to write "Denial of SB-1" on No. 31 on DS-156 ?:Have you ever been refused a US VIsa ?

2) Also, Green Card and Re-entry Permit will be considered as US Visas?

If so, I will have to write those on No.20 on DS-156 ? : Have you ever been issed a US visa?

But I can't find the types of visas on those... like F-1, K-3 etc..

Please help.

question (2) I meant question No.30 on DS156..........not NO.20
