i wasn't sure where to post this but thought i would get better results since my question is morocco specific. i'm finishing up my husband's aos paperwork and noticed the part where it says we need to include the I-693, Medical Examination of Aliens Seeking Adjustment of Status.

in morocco when he had his exam, i remembered people here saying he must demand a copy of the vaccination form. i have it in front of me and it not a i-693! it is a "vaccination documentation worksheet" for use with DS-2053. can this be used in place of the I-693? did anyone else have this situation? help!!! he went to see dr khadija (like most everyone i think) :help:

my husband didn't bring an i693a with him either. he just sent in the ds 2053 with his aos paper work and never received an rfe or anything. he later got an i693a from a civil surgeon before the interview because he wanted to be extra careful. they never asked for it at the interview, but he offered it at the end and the interviewer took it, so it's hard to say if it was really necessary or not. (he had to call several different civil surgeons before he could find one that would just do the supplement form and not the complete physical, and only charged $5. others wanted $100 :rolleyes: )

You need to take the copy of the vaccinations to the civil surgeon and then they transfer it on to the 693. Check pricing first bc they all have different charges. This is another money collecting thing bc all they do is write it on a different form.

My husband had the same issues, he had all his shots but not proof (and I'm not even sure they would have accepted proof from Algeria). We went to a NYC Dept. of Health immunization clinic and got all the shots for free (It only ends up being like 3-4 shots b/c the rest are not age appropriate). We then took the vaccine record from the clinic and brought to a civil surgeon who transferred the info to the 693A for $20.

IF USCIS accepts it that's fine. However, most just take the form you have and see the Civil Surgon here [stateside] to ahve the information transferred to the I-693A. Depending on where you are ans whom you see it could bw as litle as $40 to transfer the info.

DS 2062 is a department of state document given by a panel physician

I-692 is a USCIS document given by a civil surgeon

USCIS will accept the medical given as part of a K-1 by the panel physician if it is less than 1 year old for AOS. The medical is in your husband's immigration folder (remember the sealed envelope at POE?)

some people have sent in the DS vacciantion supplement without problems. Others have gotten a RFE for the I-693A.

I suggest you call the civil surgeons in your area and ask who will complete the 1-693A (vaccine supplement not 1693 the whole medical). Ours cost $15 dollars. They will transcripe the info fromt eh DS form to the correct form and igve it back in a seal envelope. Include this seal envelope with your aod application (noting it on the cover letter.

This is not a Morocco specific question btw. All embassies/consulates use panal physicians and use the DS form for the visa.

Any suggestions about how to get information about vaccinations already received? My husband says he's had all of his, but he has no proof of that.

If you can get a copy of his medical records, that will serve as proof. Otherwise a doctor can test his blod for the vaccines. This can be somewhat cheaper than dupllicating all vaccines. Often it is easier just to get the new set of vaccines.

When my husband and I went through the K-1 process we didn't know about asking for a copy of the vaccination report but have reminded many others about this. We filed our AOS paperwork without it. We received a RFE for it and went to a civil surgeon that same night and he had his shots again, two of them and the report was filled out and we put it back in the mail that night. They told us they go by age as to what shots are required. All in all cost us about $132 for everything but it was worth it instead of trying to get the records from Morocco. They say you do not lose "your place in line" so we acted quickly and all went well.

thanks everyone! i'll give the local civil surgeons in the area a call to find a good price! :D i'd rather not risk sending in the form from morocco in and get an RFE.

Hicham came without his vaccination worksheet so his sister sent it and we went to a civil surgeon and for $10 they turned it into the I-693. We sent that in and had no RFE's. Make sure you find someone who doesn't want him to have his exam again since he had it within the last year. Good luck!

Hi--Hamid and I are in the same problem. I ended up calling the doctor that gave him his physical in Casa. The guy gave Hamid a paper that looked like a prescription pad paper and on it it looked like he had listed two shots. That was the only paper he had from the doc, other than a tiny chest xray. He brough this to the USA with him.

SO this week I sent it to the civil surgeon that everybody here recommended on VJ in Austin, TX. Sent $10. with it. Talked to the nurse on the phone--she was wonderful. She says they have a 24 hour turnaround time. SO I am hoping I get the right form back quickly. I will keep you posted. His name is Dr. Lee, I think. And I think you can find his address and phone number on this site. If not, email me, I will dig it up for you. I left a message on the phone and the nurse called me back that day.

When I get this paper, we will have everything we need to file his AOS. By the way, he arrived Sept. 29, we got married oct. 8. What about you? Talk to you soon, Debbie

Some of the terminology used in this thread is pretty poor. Sorryif anyone takes offense to this however, so much of the verbiage is incorrect, and being that this thread also now shows up under google searches, corrections need to be made (of course there is probably some that could be corrected of my reply as well, which is why the AOS forum is best for this kind of post)

Sereia - you asked specifically about not having the I-693 (I do not know any immigrant who would have this document in hand past the POE). You got answers regarding the "Supplemental Form to I-693". I guess this is what you were referring to (note there is no such thing as an I-693a).

* There is no way to bring an "I-693" with the immigrant, this isa form used by US civil surgeons.

You need to take the copy of the vaccinations to the civil surgeon and then they transfer it on to the 693

* There is no transferring data onto a 693. The I-693 is the actual medical.

I-692 is a USCIS document given by a civil surgeon...DS 2062 is a department of state document given by a panel physician

an I-692 is a denial letter, I have no idea what a DS 2062 is.

a civil surgeon would have transcribed the vaccines to a Supplemental Form to I-693, not an I-693
