Sorry if these questions have been asked already..but on another filipino website, it says that my husband has to have a supplement letter filled out by a doctor, with his immunization records, and sealed in an envelope for us to send with the AOS papers, but on this website it says we only need his shot records from when he was at the medical examiners in the phils. He got his TD shot here, so do we make a copy of that record or does the doctor who ordered the shot have to write a letter or do we make an appt to a dr that the USCIS has approved? Also in that other website it states that the filer and the spouse have to fill out G-325 forms again, I thought it was for the intended immigrant only...do I have to fill all of that out ordo I jsut fill out the I-864 and supporting docs? My daughter moved out in August so I don't include her the I-864 right? Even though she was included when we first applied. This will effect our status on the poverty level. I make exactly $24,588.00

as always thanks for any help or advice you can give me. We are hoping to file by tomorrow but we are really wanting to avoid the dreaded RFE's......

Claudeth needed a tetanus shot, they didn't have it at St. Lukes. I took her to a civil surgeon and they did the shot and also the I-693A supplemental. I suggest you shop around, the cost can vary by a lot:

Take a look at the AOS guide here on VJ, it worked perfectly for us.

You are talking about I-693A supplement, you take vaccinations records to a US civil surgeon in the USA and get the records transferd to the supplement. This is detailed in the I-485 directions:

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report.You may include the vaccination supplement with your adjustment of status application.

Only the foreign citizen needs to provide a new G-325A

The only thing needed from US citizen is an I-864 or I-864EZ.

thanks so much! I am so glad I asked because I thought we only had to send in his immunization records withought seeing a civil surgeon because he had the medical exam in the phils. I just called one place on the "list" and they told me the office visit and paperwork is $200 plus he has to have blood work, a TB test, and possibly other immunizations. I work in an Internal Medicine office, and he's already seen a doctor I work for. He would do this for us for sooo much less but he's not on the list. How depressing! but its better to know now rather than getting an RFE later.

This is a civil surgeon registered with uscis and he will transfer the vaccination sheet your husband got from his medical in the phils to the needed I-693a for just $10!!! give them a call so you know what docs and info you need to send them. they will send everything back to you and the form will be in a sealed envelope!

Bring a copy of the I-485 directions and point out Number 8, and insist on doing just the supplement, fee for us was $30

No the Doctor is trying to make more money..... Your Husband does not need another medical.... all he needs is the information transfered from his vaccination record to the I-693A.... call another doctor in your area or use the one recommended by Nina+Mat...

He won't need a physical, if it has been under a year since his medical in Manila...They are just trying to make money off of you...We used Dr. L. Arnold too and had no problems...They were very good in getting the 693-A sent back to us..... Good Luck!!

The form from St Lukes, the black and white one you will need a copy of that. I believe your husband should have had all of the necessary shots in St. Lukes except the tetanus. You can bring your husband to your regular doctor and he can get the shot. The doctor will write proof of the shot on a form or a prescription form . Include that with the copy of the immunization forms from st lukes. When you provide this to the Dr. LE arnold he will transfer to the I-693 A form , he will mail you one in a sealed envelope, you send the sealed envelope with the AOS forms. As far as the G-325A, this is my suggestion, you are applying for the AOS NOW so your daughter doesnt live with you anymore. You should just fill out the form stating you are just supporting your husband, because that is actually the case. I didnt get any RFES at all . Good luck sis!

Just a thanks to those of you who recommended Dr. Arnold. I was actually a little over excited and mailed my husbands immunization records by FED-EX next day air, it cost me $46 to ship and had I checked with the office I would have known they were closed the next day, lol...but it still saved me the $200 plus amount that civil surgeons in my area wanted to charge us.

Now we are finally ready to fill out the rest of the paper work, but I am confused about the I-864 form, which box would I check off? They give me an option of a-f and it seems like I would check box d, but I am the only sponsor, Im not a joint sponsor, so that has me confused.
