When it comes time to file for your AOS, all you need to do is include the vaccination worksheet in place of the I-693a form that they request. It will save you time and trouble.

Mind, there have been reports that some cases are getting RFE's for including only the vaccination worksheet instead of the I-693A form - I think Krikit is referring to the other numerous cases where submitting the worksheet has resulted in an approved AOS. Personally I didn't want to risk the RFE (and getting thrown to the back of the line!) so I searched high and low for a civil surgeon. He submitted both a copy of the DS-3025 and the I-693A in the envelope. Now that's #######-retentive. ;)
