help on this? My SO arrived here Sept. 30, we were married the next week. So we sat down to fill out the I-485 Application to Register Permanent Residence or Adjust Status form.

We think we have done pretty well until we got to the medical part. I see where it says we should have a I-693A--Vaccination supplement and someone also said I-693. Well, the only things my SO were given were:

1. A "Cerficate Medical"--that is written in by the doctor. It evidently lists the several shots he had. It is written on paper from his office and at the bottom has his name and "Panel Physician for the American Consulate General". There are no numbers of any kind except his phone and address. We had to call and ask for this.

2. He was also given a Card that opens up and has a small pic of his chest xray on it. Again, no numbers or anything. It says that the imagines are normal. He was given this at his interview.

Can someone please tell us what we are to do with this stuff? Do we have to find one of those civil surgeon docs in Knoxville to fill out some forms? What forms? He was given nothing out of the infamous "brown" envelope iat JFK.

What do we do next? We appreciate any and all advice!!! Thanks, Debbie

Don't worry, the 'infamous' brown envelope was for immigration officials use only; there was nothing in it that you were supposed to have. It contained all of your immigration files from the Consulate - including the medical exam reports that your now husband handed in at his K-1 interview. What you now need for your AOS is to take the paper he did receive from the doctor showing what vaccinations he received to a civil surgeon here in the US (check the USCIS website for a list of civil surgeons in your area) and have him transfer the information to the I-693 A "Vaccination Supplement". The civil surgeon will provide this form - you don't need to do that. This has to be done in the US and would not have been done at the original medical exam. You may have to check around to find a civil surgeon who will only do the supplement - most of them will try and tell you that you need the complete medical test, but you don't. If your husband still needs any vaccinations he will need to get them done before the civil surgeon can complete the I-693 A but you can get them done anywhere as long as you have proof they were done.

He can take the card with the xray to his new family physician here in the States and they can add it to his medical file/history, or he can keep it as a souvenir.

We didn't have much luck at finding a civil surgeon in our area who would just transfer the vaccination info from the Vaccination Documentation Worksheet that we got in her medical for the K-1 visa onto the I-693 suppliment without trying to give uneeded vaccinations and charging an arm and a leg.

So in the end I followed the advice of a few other posters here and first called and subsequiently sent the worksheet we had out to Dr. L.E. Arnold in Texas who copied everthing over and sent it back to us within about a week and for only $10. Here is his info:
