can anyone recommended good USCIS Civil Surgeons in DC area???

Hi!

I've decided to get my wife's DS-3025 transferred to the I693 Supplemental form since I've seen some successful, but also many RFE, cases for AOS applications sent in without the supplemental from. So since it seems that various Civil Surgeons charge wildly differing fees and some just flat out refuse to do it, can anyone recommend a good Civil Surgeon in the DC area who will charge a reasonable for transferring the form? Maybe it is possible that someone might also know of a good one who would accept a fax of the form and then return the sealed envelope it by mail? Any recommendations are most appreciated!!!

8-15-2007-Asked embassy via email for an interview prior to APEC conference closure, they agreed

08-23-2007-Interview had and visa issued. Embassy people were really nice

09-14-2007-Entered U.S.A

09-15-2007-bought handgun, ate hotdog, drank budweiser, went to baseball game while listening to ipod

11-25-2007-MSC receives AOS, AP, EAD package

11-30-2007-NOA1 for AOS, AP, EAD

12-13-2007-RFE asking for W-2s and tax info despite my statement saying I haven't filed in years

12-18-2007-RFE answered via express mail, they received it 12-27

1-9-2008-Email received saying that case is being transfered to California (good news I think!)

1-12-2008-biometrics appointment in Alexandria, VA

1-21-2008-Advanced Parole approved

1-29-2008-EAD approved

2-14-2008-Another RFE, asking about TB skin test and saying another medical exam is required : (

2-28-2008-New medical exam completed ($190) and RFE sent back to them via express mail.

4-7-2008-Card production ordered

4-10-2008-Welcome and Approval notices sent

4-12-2008- Welcome notice and green card arrive! : ) 1 year, 2 months from K1 filing to green card

DR L. E. ARNOLD out of Austin TX is a Civil Surgeon that will do what you need for I693A form sealed in envelope by mail and for only $10.

Not sure of number off top of my head, but call his office monday, leave message and they will call you back and then tell them what you are needing to do and they will give you directions.

Look his number up under Civil Surgeons on web or there should be his name, address and number on one of the AOS forums, just do a search at the bottom of page and type in I693A medical form or I693A fees or Dr L E Arnold and then you will find it, I posted it several days ago and simply do not have the number in front of me handy now, but it is already out here on this AOS forum under I693A fees recently in past few days. Good luck and yes I even used this guy after calling a local Civil Surgeon office wanting to charge me $200 to fill out the form! Highway robbery!

Hi there, I am from McLean, VA and I did not have any problem with the Civil Surgeon transferring my vaccination records into the Supplemental form (NO RFE). I don't remember how much was it but for sure it's less than $35, am not just sure if they can do it thru FAX. You may call them for information.

Call them monday morning, they open at 830 am (remember that texas is 2 hours behind). they will let you know what you have to send in and it really just costs $10. i jus did it myself recently and had no problems.
