Lisa:

do you want to change your last name?

You do NOT need to change your last name because you got married. It is not preferred by anyone official and does not mean you are less married if you do not change your name.

I did not change my last name at marriage. My husband was thinking of changing HIS, but that is another story.

You have to tell us if you WANT to change your name. If you do, there are steps to take.

If you do NOT want to change it, you are not required to do so.

Of course you do. If you are using your married name, your signature needs to match it. Just practice writing it a few times before you do the form so that it flows smoothly... your signature IS your name :)
