Does anybody know where I can find the I-693A vaccination supplement needed with the I-485?

I can't seem do find it.

The US Civil surgeon should have this. You bring vaccination records to the C.S. and have the record transfered to the supplement.

hey, i just dealed with getting the I-693a! it is indeed like jasman0717 said, you need to call around to find a civil surgeon that will transfer your vacc supplement without wanting you to get the whole medical done again! which you dont need when you apply for aos within one year of your original medical!! or they will offer to just transfere the vaccinations but for the same price as the whole medical, mostly $125, whats ridiculous!!

anyways, there is a doctor in texas that will transfer the original vacc supplement to the I-693a for just $10! just give them a call and theyll tell you what you need to do! its really easy, i just did it myself! you just have to be patient a little because of snail mail......
