Question on Medical prior to filing

So I am refiling my papers and stating again after a mess up of the first process. But anyway, the doctor this time round wants me to have a chest xray for CXR. I dont remember doing this on my first medical for my first application, is it really necessary because im sure its not cheap

A chest xray is required to check for TB. Sometimes the CS here in the States also wants to do the skin test as well.

Is this the AOS medical or for a K-visa medical?

Depending on where you live, you could always use the USCIS civil surgeon locator to call around and see whether you can get a medical exam at a better price somewhere else. $400~ sounds really steep.

YOU ARE GETTING TAKEN TO THE CLEANERS!

all you have to do is mail your DS vaccination form(you got from first medical exam in your home country)to this Dr here and he will fill out for $10, he is Civil Surgeon.

That's assuming that the OP came over on a K1 or K3 visa. If he didn't, then he'd have to do a complete medical examination, no?

Yes, if he hasn't had a USCIS medical examination in the past year (which he only had, in his home country, if he came as a K-1 or K-3), he needs the full examination. It cost me $200 in NYC, including all blood tests and chest X-ray.
