Case by case varies.

All depends on the evidence you have of proving a bonafide marriage and if your documents are in order.

Multiple trips by the petitioner helps as they most always ask how many times you've met.

Not required. But the petitioner attending interview helps also.

Also seems like they will allow you to pick up the visa more often if the USC petitioner is present.
