DS-230 Coversheet Urgent help needed!!

Please, I need everybody's comment on this. The DS-230 coversheet. Does it only contain the case#, the barcode and CR1/IR1 at the bottom?? OR does it contain case#, the barcode and CR1/IR1 at the bottom AND the agent's name and the embassy you are going through???

Did anyone recived the DS-230 cover sheet in last few week?? How did it look like?? I am using the james shortcut and I am geeting very confused. Please help. Thanks all!!

OR, are you talking about a coversheet for an RFE (checklist) for the DS-230?

I have not heard of any changes with DS230. If you follow the James Short v2.0, it is still the most updated one out there

The coversheet I received was exactly like the one found in James' version 2. The barcode is on top of the page (left hand side) and the case # is at the bottom.

05/10/07 - NVC receives petition, case # assigned

08/08/07 - Case Complete

09/27/07 - Interview, visa granted

10/02/07 - POE

11/16/07 - Received green card and Welcome to America letter in the mail

Removing Conditions

07/06/09 - I-751 sent to CSC

08/14/09 - Biometrics

09/27/09 - Approved

10/01/09 - Received 10 year green card

U.S. Citizenship

03/30/11 - N-400 sent via Priority Mail w/ delivery confirmation

05/12/11 - Biometrics

07/20/11 - Interview - passed

07/20/11 - Oath ceremony - same day as interview

Thank you so much everyone and especiially Diana, I got your reply! I was looking for DS-230 coversheet, and I was looking for the old version and that's why I was confused. Now I got it. Good luck to all of you!!

By the way, just to make it exacly like that, I am asking this. The name and address at the top, should it be with the all capital letters OR just the regular letters as James put there?
