Hi Pari,

Please double check that you have the latest version of the DS-230 - it should have an expiry date in the top right hand corner of 09/30/2010 (they just very recently updated it).

I am filling out my form DS-230 as of now and I am confused on Question #2 my full name is such:

First: Pari Last : Walia middle : Mann

now that i got married to my husband who has same last name as mine..i didn't have to change my last name to take his and my delimma is for Question # 2, what should i write in that?

Write your maiden anyway, even though it hasn't really changed.

Also, question # 22 because i have only worked for two places for about only 2 years since i m university student and didn't work as of now that much!!! Will they question that how come i didn't work that much????

No they won't, just write what you have.

Also, question # 23 are we suppposed to write all educational institute because i came to canada when i was 15 and so i studied until grade 9 in my home country so should i also wirte down the adress of my home country schools?

For my Husband we listed ALL schools, right from primary through post secondary. I have read some posts where people only put like high school and university.

In my opinion you should list all schools, including the ones from your home country.

Also, Question # 25 because i went to USA many times and i don't remember all the dates ...and even if i remember some dates but they are approx. further is that in which categorized visa should i put since i m canadian citizen and i don't need to get a visa at border???!!!

List as many as you can remember, enter the dates that you remember, just be as accurate as you can. For visa type I would put VWP (visa waiver program).

For #25, I added an attachment with the exact headings required:

From To Location Type of Visa "A" number

If I got stamped in my passport I indicated B2* under visa type and the lower down on the page the asterisk: * B2 stamped in passport at airport.

If I didn't get a stamp, I put down Visitor/Pleasure** under visa type and lower down the two asterisks: ** Entry at Canada / U.S. border or POE at airport, no stamp in passport.

For unknown dates I just put down an approximate month and year. For tons of childhood trips I put down the years like so: Multiple visits over summer vacations as a child from 19xx to 19xx
