I'm filling out some P3/P4 forms for my SO in China.

Forms that initially ask for a name clearly label the boxes with Last Name, First Name. But on some of these forms, they ask for names of children or other relatives and just provide a line. Have other people had any trouble with name order if they enter names American style (first, last). Should I use Chinese convention (last name, first name)? I'm concerned about a government data entry person entering the wrong info and causing delays in processing.

Also, I've decided not to use "none" in the "Middle Name" spot and to leave it blank instead.

Any comments about potential name entry problems would be appreciated.

I also had a bit of trouble filling in the names on our forms. My fiance's native language is Arabic, so things are read and written (including filling out forms) from right to left. When he writes his name in English, he always writes his surname first. But when I filled out the forms for our visa I entered given name first and surname last (unless specifically prompted by the form to do otherwise). I entered our names, as you say American-style. For his middle name I also put "NONE" on the form, since he has no middle name and the instructions indicate its better to put "NONE" rather than leaving a blank (or N/A as the case may be).

Hi there... Did you ever get a reply from the consulate? My fiance is Vietnamese and we are debating which way to display the name. I think we are going to use the US version in the forms (first, middle, last), but then have a letter explaining how Vietnamese names are ordered and that there are some documents which will reference her name last, middle, first.

I am planning to include something like this in my cover letter... Do you think this will be sufficient?

Please note that the name for the beneficiary is ordered according to US standards (first, middle, last name). The beneficiary is from Vietnam and in Vietnam, names are ordered as (last, middle, first). Please keep this in mind when reviewing some documents used in the proof of having met which refer to her name in the Vietnamese order.
