Here I am again with a question that probably been asked here before million times,but still I want to make sure!

Today I went to my local doctor for all check ups and i also asked them about vaccination form that is required for adjustment of a status.For my surprise nobody has a clue what I am asking for.So here is my question:Once I have got from IOm centre vaccination sheet, what else I should get for adjustment paperwork?

How that form is called?Do I need a lab works to do for them to approve vaccinations I had done?Or i just have to dounload somewhere the form and ask them to sign it?

You need to find a Civil Surgeon -- there are lists of them available at government websites. These are doctors who will evaluate your vaccination status and provided the information needed for a form (I think it is I-693A but I'm not sure -- we're not there yet) that you will file with your AOS packet.

Now, with these Civil Surgeons, be careful. Some will try to give you vaccinations you don't need. There are examples of this sort of thing elsewhere on VJ if you look around. If you don't mind extra vaccinations, no problem -- but some vaccinations may have side-effects and you might want to be careful.

This fellow is mentioned elsewhere on VJ as a hassle-free way to get your I-693A taken care of, provided you have your vaccination record from your medical in Moscow. I do not know this person and have not contacted him.

All you need is a record of what vaccinations you have had.

The big ones are MMR(measles, mumps, ruebella)

Chicken pox

Polio

Truth be told when we went to the local civil surgeon they filled out the form and we went and picked it up in a few days there were a few vaccinations we couldnt get because of pregnancy but at the AOS interview the CO didnt even ask about it. We didnt even get RFI'd for it.

The I-693A is the correct form and you must use an approved civil surgeon. Not all Dr.'s offices have a civil surgeon so you may have to check around.

They could try to give you more shots and they may also try to give you a complete exam (for more money!) so make sure you tell them you only need the I-693A form filled out, not a physical, not a complete examination, only a vaccination supplement. THAT'S IT!!!

Ensure your beneficiary makes and brings with them to the States a copy of the DS-3025 (vaccination form)

If the government is going to force me to exercise my "right" to health care, then they better start requiring people to exercise their Right to Bear Arms. - "Where's my public option rifle?"

Check the USCIS web site for the Civil Surgeons in your area. In my opinion, this is a scam operation so be very careful with the doctor you choose. Some charge a pretty steep fee and don't do much except complete the paperwork. So, ask questions first, pay second.

We argued with every dam civil surgeon in our area. No possible way to get i without another full examination. Some people have all the luck and breeze right through this. My wifes examination from Moscow is on a ds-2053. I thought it was on a i-693. No ds-3025 was recorded in Moscow. Civil surgeoons here could not translate her immunization record and didnt believe our word for it. I cant even put a word on how pissed of I was (still am) about this. Probably 10 different civil surgeons-same story. Last ditch effort she found a Russian civil surgeon by contacting the owners of a Russian store by where we live. Thank God

No problems! We went the same day, no appointment necessary since my wife is Russian the doc told her. She gave her the shots she needs, translated her immunization record, filled out her i-693 and i-693a and we pick it up tomorrow signed and sealed for our interview in a few weeks. I cant believe the bs we had to put up with.

All I can say is good luck and I hope you dont have to go through what we did..

That's pretty cool!

I wish we could find a Russian doctor near our home. My wife doesn't seem to think the doctors here have any idea what they're talking about. Maybe at least if they were speaking Russian to her she would believe them then. I guess we'll try the Russian store also.

I never thought we'd find a doctor next to the saira.
