do we attach picture to DS 230?

Hey I'm not sure if i'm supposed to attach my husband's picture to the DS 230 form. Am I?

and about the fee... when do you pay it? i'm going with my husband to the embassy here and we're turning all our paperwork into the VFS office, but there was no mention about paying a fee then for the forms. they told me that i was to pay a buttload of money at the interview.... which is correct? thanks! buena suerte
