I believe, not 100%, that our doctor used the I-693 without the "a" and just wrote all my vaccination history in the box that said "history transferred from other medical form" or something (dont quote me on the title box). I have been reading about people having stuff checked off on their forms and stuff. Could you tell me exactly how your civil sugeon complete your I-693(A) because now I am kinda worried they did it wrong. I figured they knew what they were doing.

This is how the Civil Surgeon did it on my I-693A Vaccination Supplement Form:

The box that is entitiled: "Vaccine History Transferred from a Written Record," the Dr wrote 'Patient brought vaccination records from the UK.'

Below that there is a list of various vaccines with several boxes for dates, contra-indications etc. The Dr. ticked these off or entered a date that the vaccine was received for each one. eg Polio is checked as 'Not age appropriate' and for the Td vaccine she entered the date I had the shot 02/21/06. I had some titers run for the MMR and the postitive titer results and dates were entered under the Fully Immune column.

So basically there is either a note, a date, or a check in a column for each of the vaccines listed. I hope this helps you. We had to go back to the Civil Surgeon about 4 times to get various things added or corrected ie. missing Alien #, wrong address etc. Apparantly she had been doing for 10 years too and I guess it might have been okay, but we wanted to make absolutely sure that USCIS would not send it back as incomplete.

7th April 2006 - EAD arrived in mailbox.

21st April 2006- Received Interview Date for 22nd June

9th May 2006- Received SSN

22nd June 2006- AOS INTERVIEW APPROVED

REMOVING CONDITIONS

June 20 2008 - Package mailed to CSC under new rules. Would have been an NSC transfer

June 23 2008 - Package recieved at CSC

June 27 2008 - Recieve NOA1

July 16 2008 - Biometrics

July 17 2008 - Touched

September 9 2008 - Card production ordered

Make sure he checked off if ure positive or negative for syphillis and HIV as i was not able to get approval on my interview last monday because the doctor didn't check anythin.. He only wrote what types of tests he did in regards to that.

I have a feeling i should get it redone... i mean, its just been a nightmare since day one.

I got these people off of the USCIS website so I called them instead of driving an hour away to find out that they wont\cant do it. So i was transferred around to 5 people where i explained what i needed and finally the head nurse said they do the supplement its 25 dollars and to just walk right in, no appointment needed. So the next week my husband and i drove an hour out of our way, I walk into the clinic and told them what i was there for. The secretary had no clue what i was talking about. She went to talk to the head nurse and doctor and explained and she said they couldnt do it and they gave no explanation as to why. I told her i called and was told they did it. She said, "Well yeah we do INS stuff but I dont think we do this..." and I am thinking "you idiot, the vaccinations IS part of the INS medical!" And, of course they couldnt get ahold of the head nurse yadda yadda. Long story short, they ended giving me the head nurse's number to leave a message and i went home empty handed. I called the head nurse and left a message and told her how unimpressed I was that i had called to confirm this could be done and drove out of my way to be turned away with no explanation. The head nurse called me back and told me she would like to know who i talked to at the clinic because she would like to reprimand them for sending me away because they do indeed do the vacciantion supplement. She talked to the secretary who admitted she was only half listening when the head nurse had called and explained my situation and said I would be coming in sometime the next week. She ended giving me a 15 dollar discount and apologized and told me she notified them that I would be back the following week, so i went back. I get there and get called into a room. The head nurse at the clinic (not the one i was on the phone with), comes in, sits down, and she says to me "yeah i never do this INS stuff but i think i know what to do..." oh my gosh. She wrote down all my history, but checked nothing off. She had me written as born in the USA and had my birth date as 2006, i had to correct it all for her. Then she kind of stared at it and said, "I'll be right back," and left with it. 45 minutes later she comes back with the form and told me that her and the civil surgeon (yes, he didnt even fill it out himself, he just signed it) had to call the head nurse i had talked to on the phone because they needed to ask her a couple of question regarding the supplement but they think they had it all figured out now. So she put in an enveloped, taped it, wrote my name and "INS" on it and sent me on my way, very confused and unsure if it was done properly.

Anyway, didnt mean to ramble on but it was such a huge mess i am wondering if should just get a new one done somewhere else.

Stina, i have never heard of a medical done for only $25.. WE paid $200, and everyone else we read about paid around that much..!

Also, you didn't mention anything about them withdrawing blood from you to test! HOw is that possible?

And also, what we did to know if the doctor was approved by USCIS, is that we CALLED USCIS and they gave us a list of the closest doctors we could go to. We had to drive 1/2hr away out of TN state to get to the closest doctor they gave us..

So how did you find out that your doctor does INS and all?

Hmm, something sounds fishy... I wonder what ur reaction was when the nurse said "i've never done this b4, but i think i know what i need to do!" lolol

I didnt need the medical, but only the supplemt, which the going price seems to be anywhere between 25-45 dollars. I didnt call the USCIS but went to their website where they had a bunch of approved places that take care of INS stuff. Before I went the second time, i asked the head nurse on the phone if there was going to be an approved INS doctor there the day i was planning on going and she checked the schedule and said yes. I figured they knew what they were doing, i am starting to think not.

Oh my gosh, when the nurse said that to me, my heart went into my stomach, it was one of those situations where someone says something and you just dont know what to or say... but if i had said something, i probably wouldve gotten deported lol

I checked your info but it didnt say, did you arrive in the US on a K-1? I did my full medical back in Canada as it was mandatory to be approved. The medical is good for one year and all that I need to do for my AOS is take the vaccination worksheet with my info on it that the surgeon in Canada gave me, and have a surgeon here transfer all the vaccination information onto a I-693A and send that off with my packet.

I did not have my A (alien) number on the form. I filled out the top portion myself -- with name, passport #, etc -- and I don't think I knew then what the field was for. I had no problem at all at the interview.

09-02-2005 Applications for AOS, EAD, and AP received by MSC

10-21-2005 AOS fingerprint notice for 12-08-2005

11-07-2005 AP approved

12-05-2005 Infopass appt at San Jose office for interim EAD -- Refused, because it is already approved by MSC on 11-07-2005

12-07-2005 Attempt at interim EAD at San Francisco office -- no go. Back to San Jose, where CSO (chief station officer) tells they will contact MSC via email to request permission to issue interim EAD

12-08-2005 Biometrics for AOS and EAD. Having no EAD appt letter was no problem (used EAD NOA)

12-15-2005 EAD arrived in the mail

12-24-2005 Received interview letter; interview scheduled 03-01-2006

01-28-2006 Received replacement SSN card in married name (5 wks since application)

03-01-2006 AOS interview -- approved; received stamp in the passport

03-13-2006 Green card arrived in the mail

Stina, as I see it you have a couple of options. Either submit the form as it is and hope, or if you have a really negative feeling about this place, which I'm sensing you do, I would get the supplement re-done elsewhere. If you choose the latter, you can open that one up and see how complete it is. Like I said, I had a hard time getting mine correct, but I kept going back to the office and getting them to correct it. I was fortunate that they gave me a copy of both my medical and the Vaccination Supplement, so I could check the accuracy of them. I did not open up the sealed envelope myself, but when I saw the errors on my copy (first one was that they put completely the wrong town for my address) I was able to take it back to them and just waited for it to be corrected. I was also fortunate that they did this willingly since we'd paid them already. However, if it is a matter of $25 or so and you can find someone else to do it, I would get it re-done.

NB: The part that states: "After the civil surgeon administers any needed vaccines, he or she must complete Supplemental

Form To I-693, Vaccination Record, and for adjustment of status applicants, also complete Form

Thank you ktun!

My husband and I decided it best we go to Dr. Miff, who we originally going to go to as he was recommended to us from someone on here. We went to this other clinic cause it was a lot closer than chicago. But, just the fact that these people had no clue what they were doing form day one has my mind very unsettled and dont want to risk an rfe. I will be calling the head nurse agin to tell her about my experience with filling out the supplement at the clinic.
