Hi All

For those member here in VJ that they already done with their AOS paper work.I really appreciate your assistance in answering my questions below.

1. For Form I-485

a.) Where could i get the Alien No.?

b.) Could I send the I-485 without my SSN and leave as blank? since i've been waiting more than a month for my SSN but till now i have not receive yet.

2. here are the forms i got ( I-485,I-864, I-765 and G-325A), is there any additional forms to submit?

3. The I-94 form that stapled on my passport. do i need to send the original I-94 or they only need the photocopy?

4 I have already done with my TD shot. the Doctor gave it to me the vaccination record. Do i need to send the photocopy of my vaccination record?

It usually is found on your K-1 NOA2. It may also appear on your visa (printed when the visa was issued, or handwritten by the immigration dude at your point of entry).

Not a problem. Provide it to them if you have it, but the social security number is not required to file for AOS.

Nope, that's it, unless you wanted the advance parole document, in which case you should also file I-131.

The I-94 form that stapled on my passport. do i need to send the original I-94 or they only need the photocopy?

Do not ever detach the I-94 from your passport. Send them a photocopy.

When you have your interview, and if you are approved, the gov't employee will take your I-94 himself.

If you do not have to go to an interview and are approved, you may discard your I-94 when you get your green card.

I'll leave your question 4 to someone else as I'm never too sure about the whole medical aspect of the process.

2007-02-26: filed for K-1

2007-03-19: K-1 approved

2007-06-11: K-1 in hand

2007-07-03: arrived in USA

2007-07-21: got married, yay!

2007-07-28: applied for green card

2008-02-19: conditional green card in hand

2010-01-05: applied for removal of conditions

2010-06-14: 10-year green card in hand

2013-11-19: applied for US citizenship

2014-02-10: became a US citizen

2014-02-22: applied for US passport

2014-03-14: received US passport

1 b. no need for SSN before filing for AOS

2. NO unless you want an AP document to travel out of the USA then I-131 also.

3. Send only photo copies.

USCIS no longer routinely requires submission of original documents or "certified copies." Instead, ordinary legible photocopies of such documents (including naturalization certificates and alien registration cards) will be acceptable for initial filing and approval of petitions and applications.

Bring the origionals to the AOS interview.

4. You need to get your vaccination record transfered to a supplement by a US Civil Surgeon per the I-485 directions.

your question #4, your vaccination must be in a sealed envelope and not just a photocopy, and done by the designated surgeon of the the uscis. however, you can bring your vaccination record to a designated surgeon by the uscis to transfer to the right forms and be sealed in an envelope before submitting.

ANSWERS TO YOUR FOLLOWING QUESTIONS

1. For Form I-485

a.) The Alien No is written on your I-94 which is attached to your passport it start with "A" written at the back side of your I-94 as i also had in my passport at the port of entry.

b.) Could I send the I-485 without my SSN and leave as blank? since i've been waiting more than a month for my SSN but till now i have not receive yet.

b.)Yes u can as u had an A number on your I-94 that is very important

2. here are the forms i got ( I-485,I-864, I-765 and G-325A), is there any additional forms to submit?

2.No the only form u need that is to apply for advance parole means if u had to go outside usa before green card otherwise no other form is required

3. They only need the photocopy dont send the original it should remain with passport

FEDERAL REGISTER INTERIM REGULATIONS at USCIS website, I quote:

"C. Medical Examinations

According to 8 CFR 245.5, aliens seeking to adjust status are required to undergo a medical examination performed by a designated civil surgeon to determine whether they are inadmissible under section 212(a)(1)(A) of the Act. To date, applicants for K nonimmigrant visas have been required to obtain a medical examination abroad pursuant to Department of State regulations at 22 CFR 41.81 prior to entry, and the medical examination is not repeated if they apply for adjustment of status within 1 year of the dat e the examination was performed. They are, however, required to submit with the adjustment of status application a vaccination assessment completed by a designated civil surgeon in order to establish their compliance with the vaccination requirements under section 212(a)(1)(A)(ii) of the Act.

its the A# on your Visa (its written like this A#99-123-00 )

yes you can but don't leave any spave blank, put "none" because you don't have it yet.

that would be okay, you can add the Advance Parole if you want to.

For question # 4 I sent the vaccination worksheet which the doctor gave to my fiancee. The consular officer overseas asked my fiancee to send this worksheet (which was filled and signed by a "panel physician" along with AOS application in place of the supplemental form). You should be OK sending what the doctor gave you if you had done your medical exam overseas.

10-16-07 mailed AOS packet

10-18-07 packet received by USCIS, Chicago Lockbox

Thank to all you guys for info's you had provided for adjustment of status.

I just sent my packet last week to USCIS, Chicago lockbox thru USPS.

How would i check if they already processing my papers? For now i just check in USPS and it says they already received my packet.
