Immunizations and I693 Supplement

So...you didn't give them information about the shots you received from the other doctor?

I did inform the doctor asking me about immunization.. I even show her the card...

But anyways, I did visit a CS in my area..yet he is charging too much ($250)..But my husband decided to go for it just to finish everything.....

St. Luke's is still supposed to give you the DS-3025, even if they did not administer the shots.

They transcribe that information on the form for you.

Hi, I just want to know more about this form and the I-693. My interview is approaching and I am still worried about this form and the I-693. I didn't had the I-693 evaluation by civil surgeon when I submitted my application for I-485 but I enclosed a copy of the DS-3025 which I got from the medical examination at St. Luke's.

They didn't send me anything asking me to have the I-693 or show proof for the medical but now it was written on the notice to bring I-693 in a sealed envelop and Supplemental Vaccination Form unless already submitted. This is confusing, what I know is that they will send a RFI if they need more information and proof from the applicant, so do I have to have the I-693 done before I go for my interview?

I already try to inquire for the medical exam nearby and it cost a lot as usual. What is the best thing to do?? January 20th is my interview date in Jacksonville, FL.
