Hey all-

I made an appointment with a civil surgeon to fill out I693 supplement. My husband has had vaccination for Deptheria/Pertusis, MMR. According to the numerous posts on VJ, it seems he will need the varicella (chickenpox) vaccine as well.

I called the civil surgeon today to confirm the appointment and he states my husband doesn't need the varicella shot. Instead, he needs a flu shot because we are in flu season. He also stated being though my husband has had chicken pox before, he wouldn't need the varicella vaccine.

Please advise. I'm scared to listen to him and mail off everything just to receive an RFE.

12/14/07-AOS case moved to California Service Center for further processing

12/20/07-EAD approved!!!! Card production ordered

1/2/08-Received EAD card in mail.

2/6/08-AOS approved!!!!! Card production ordered. FINALLY:)

Lifting Conditions (I751)

11/3/09-Fed-Ex'd package to Vermont

11/5/09-VSC rec'd package

11/10/09-Received NOA1 with 1 year extended conditional residency

11/18/09-Received Biometrics appts for Dec. 10th (Dated 11/17/09)

2/11/10-Green Card Approved!

2/19/10-Green Card received! Glad that's over....next up Citizenship.

Both the Doctor in Canada when I went for my Medical and the Civil Surgeon here in the US when I went to get my I 693 both accepted oral testimony that I had the chicken pox.

I don't think you will have any problems.

The C.S. has the discretion in this matter, so if he says varicella not needed and the doctor signs off on the I-693A then it will be accepted by USCIS.

My husband has had chicken pox before but he didn't have any type of documentation stating that and the civil surgeon would not accept his word so we had to pay for the blood test to prove that he was imune to it. I guess it's up to the civil surgeon.

He didn't have to have the flu shot because the CS said it wasn't required.

It really depends on the civil surgeon, so it is wise to call around first before deciding on which civil surgeon to go to. Varicella vaccine is not required anymore if you had have chicken pox before, then you can just bring all your proof of vaccinations to record in your I-693 Supplement Form. A few civil surgeons are willing to just transfer your records without requiring any vaccination for a nominal fee. Don't worry about getting an RFE, because your civil surgeon will inform you if your immunizations are not complete, and would probably ask you to come back after your immunization is completed before he signs the form.

If I cause offense it is not on purpose, think about that before you take offense. If you feel offended, consider that maybe I'm not talking about you and maybe I accidentally stumbled onto a sore area and do not know. Thank you.

FEDERAL REGISTER INTERIM REGULATIONS at USCIS

"C. Medical Examinations

According to 8 CFR 245.5, aliens seeking to adjust status are required to undergo a medical examination performed by a designated civil surgeon to determine whether they are inadmissible under section 212(a)(1)(A) of the Act. To date, applicants for K nonimmigrant visas have been required to obtain a medical examination abroad pursuant to Department of State regulations at 22 CFR 41.81 prior to entry, and the medical examination is not repeated if they apply for adjustment of status within 1 year of the dat e the examination was performed. They are, however, required to submit with the adjustment of status application a vaccination assessment completed by a designated civil surgeon in order to establish their compliance with the vaccination requirements under section 212(a)(1)(A)(ii) of the Act.

The Service will continue this same policy for the K-3/K-4 nonimmigrants. K-3/K-4 nonimmigrants who file their adjustment of status application within 1 year from the date of the medical examination overseas will not have to submit an additional medical examination. However, the Service notes that applicants whose medical examinations overseas revealed a “Class A” or “Class B” condition (as defined by 42 CFR 34.2( B ) ) must establish upon application for adjustment of status that they complied with those cond itions imposed on the initial admission. Failure to comply with those conditions means that a new medical examination will be required.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

A copy of the DS-3025 will be given to you by St. Luke's after you have completed your medical exam. They will determine the vaccines or booster shots you need based on your age and gender (all vaccines or booster shots are already covered by your medical fee), and if you have proof of being previously vaccinated (like a baby book or certification from your pediatrician).

You may want to update your profile and your timeline so that when you have questions, other VJ members can help you better.

Hi guys, I already have my 9 vaccines on my process of my K1 visa but I dont have the ds-3025 form.. do i need to undergo additional vaccines? how many shots needed for a certain vaccine?

When I had my medical exam in St. Lukes I already have my vaccines given by the other MD in my place so I decided not to have shots in St. lukes anymore cos my arms were still painful for all the shots. Besides, I am also afraid during that time that I might experience fever if i will undergo another shots thinking that my interview will be on the next day...

St. Luke's is still supposed to give you the DS-3025, even if they did not administer the shots.

They transcribe that information on the form for you.

When I had my medical exam in St. Lukes I already have my vaccines given by the other MD in my place so I decided not to have shots in St. lukes anymore cos my arms were still painful for all the shots. Besides, I am also afraid during that time that I might experience fever if i will undergo another shots thinking that my interview will be on the next day...

I have here DS2053 or DS-2054.. just the from alone... nothing shots given....and there a note below saying :

K1 visa applicant - prefers to be immunized in the United States.
