Hi,

I am about to file an I-485 for my wife. She is here on a K-1 Visa. The process requires me to submit a I-693a. All I have is a DS-3025. Will this do? or should I get a I-693A? if so how do I go about getting one done?

This is one of the most common questions on VJ. The answer is not an easy one because some people have been successful with just submitting the Vaccination Worksheet from their K-1 medical exams. I was one of them.

However, you risk getting an RFE for the Vaccination Supplement (I-693A), which isn't really all that hard to get.

For my K-2 daughter's AOS, I took her Vaccination Worksheet from her K-2 medical exam and had the Civil Surgeon in the U.S. transfer the information from it and put it onto the Vaccination Supplement. They charged us $35.00 for this service.

Call around to Civil Surgeons in your area to get the best price. They vary greatly from one doctor to the next.

No you need to complete the I-693A. Its available at cdc.gov at this link http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf

You will need a doctor who has been designated as a Civil Surgeon must complete the form. You can locate your nearest CS by visiting http://www.uscis.gov/portal/site/uscis/men...00048f3d6a1RCRD

If your wife came on a K1 she only needs to have someone transcribe her vaccine records to for the I-693 (the K1 medical exam is valid for 1 year). Bring a copy of the DS-3025 just incase (immigration kept my husbands so I've never looked at the form but I'm guessing it probably has pertinent medical info on it). Also I've attached a chart which will show you which vaccines you wife may or may not need (because of age difference, not everyone needs to have all the vaccines listed on the I-693A. Some CS will try to make her do a physical or pay full price to copy vaccine records; if this happens contact Dr. Arnold (info Below) his/her office will do it for $10 by fax/mail. If you look at the attached chart and determine you wife needs additional vaccines, consider going to you local health departments, vaccines only cost around $10 dollars or so. You may have to pay full price for varicella, I couldn't find anyone in our area even willing to give varicella to an adult except for the CS...it was around $90 if I remember correctly.

http://www.visajourney.com/forums/index.ph...page=definition check this site out its VJs immigration acronym dictionary. I was on here for probably 5 months before I knew it existed! Embarrasing.

Anyway, RFE means "Request for Evidence." Basically, if your supposed to turn in something (tax forms, I-693A, etc...) with your application but don't, they will send you a letter requesting additional evidence. The letter will list specific items/documents that are need to continue processing your case. You case will be on "hold" until you submitt the info. Once you submit, processing will begin. I haven't experience an RFE, but other members have reported that their processing continues soon after the submit the requested items.

Make sure you call around, the prices for civil surgeons can vary by a lot. Good luck :thumbs:
YES and explain to the C.S. that all you want is a supplement done, most want to gouge you for a full medical exam. IT IS NOT NEEDED! Take a copy of the I-485 and point at the paragraph explaining that since you had a medical exam in foreign country for the visa, less than a year ago that the supplement is all you needed.

DO NOT WASTE YOUR time with those other posts or shopping around, REESE16 is telling you how it is, call DR L E ARNOLD and for $10 you get that I693A filled out from him and in separate envelope to mail in, thus you do not risk getting a RFE which will only waste more of your time. AND I know because from experience it works using L E Arnold and it is very inexpensive. You send in that DS form and you risk getting a RFE, your call.
