Isn't it just vernacular? I don't really pay attention to the greeting itself... just find it nice to be greeted with sincerity. :)

But is it really sincere if the person doesn't actually care to know how you are?

I've yet to figure out a proper response to "What's up?" (or as a blonde, white, female coworker likes to say, "What up, yo?"). Somehow, "Hello," or "I'm fine," doesn't make sense as a reply. I usually take it literally and say something like, "The ceiling."

depending on if they are annoying me, i either say

I was thinking about greetings coming from workers behind the counter or at restaurants where they're required to greet you with a, "Good Morning," or "Good Afternoon." Even if they add the, "How are you?" line, I don't take it literally - but rather see it as an acknowlement of my presence with their attentiveness.
