I think you should write your family name even if its not used in ur passport .and I don't think its gonna be a problem because you will have to show them ur birth certificate wich has ur family name.

I'm not an expert on Indian names but if no documents bear the family name sidhu, and you are male, I would leave it off and write N/A or just a horizontal line in the middle name field.

If you are female, you need a new passport with your married name.

Well, it is difficult....

Harpeet, are you the beneficiary or the petitioner?

If you are the petitioner and your passport is US, then go with what is in your passport.

If you are the beneficiary outside the US, I would default and use the name given on the birth certificate.

It is better to make an error by giving too much information than not enough.

now what?

Go with the birth certificate. If it shows Harpeet Kaur and no Sidhu, than use Harpeet Kaur only.

This "family name" is not relevant if it's not listed on official documents like birth certificates or passports. This is more for your locality where family and caste are an issue.

Here is a link that might help you.

Primary Documents Given Greater Weight

Generally, primary documents like the birth certificate are considered more reliable than secondary evidence like a person's passport. A document created ten or twenty years before the person ever considered traveling to the U.S. is accorded greater weight and reliability as being authentic, like a baptismal record or a School Leaving Certificate....

You are making this issue much more complicated that it actually is. My wife name is _____ Kaur. Her family name is something else. She uses Kaur as the "family name". In United States, most people have their first name and then the family name. Most people also have middle name. For Sikh names, we have the family name and then the religious last name which is either Singh, for men, and Kaur, for women. Also it is rare for Sikhs to have a middle name so it is O.K. to leave it blank. If you leave it blank, they will assume that you don't have a middle name. The immigration people don't care about what is your actual "family name". They just want the whole name as it is used on your passport, birth certificate, and the marriage certificate. This is all I have to say on this issue. It is up to you to believe it or not.
