Ok! Ok! I know! it's been way too long!! how's everyone!! I spent a little time checking up on some of the posts. Glad to see IMBRA is a thing of the past, what a mess that was. I feel like the process is different for the new petitioners, luck for them!! that's great!! :dance: anyway...

I'm finally getting ready to send the AOS in. I'm in the package formation stage :wacko: . I read about the non-us-petitioner using an I-693A supplement to the examination form if they've had an examination within the year. Well bingo!! my wife fits that perfectly. One question...... what is it? :help: does it cost money? tried looking it up on the site but their didn't seem to be a link to it. I don't see a I-693A on the USCIS website either. Can someone shed some light on this :thumbs:

What do I do? and how do I do it? what's involved with it? I have no clue :blink:

06/30/06- sent I-129F petition to VSC.

07/11/06- NOA1 (received in mail)

07/19/06- RFE received in mail, IMBRA

07/28/06- e-mail, IMBRA RFE received at VSC

09/11/06- called VSC, told I'm with an adjudicating officer

09/25/06- called VSC, told to file expedited service, outside 60 days of RFE received date

09/26/06- sent expedited service request to VSC via fax and snail mail

09/26/06- Senators and Congressman making inquiries into my petition

09/29/06- expedited service request approved by VSC director

10/04/06- e-mail NOA2 approved!!!!!!

10/05/06- NVC received

10/06/06- NVC forwarded to Poland Embassy

10/16/06- Packet #3 received

10/17/06- Packet #3 sent to US Embassy Warsaw

10/30/06- Packet #4 received

10/30/06- Interview scheduled for November 22nd!!!

11/30/06- Visa in hand, via DHL!!!!

12/05/06- Ania and I arrive together in the USA

07/24/07- Social Security Card in hand- after some minor issues and a little procrastination :)

10/14/07- I-485, I-765, and permission to travel mailed to USCIS

12/17/07- I-485 shipped to California Service Center for faster service

12/17/07- I-765 approved new card should be mailed within 30 days, Advanced parole approved

02/28/08- AOS APPROVED!!!! PERMANENT RESIDENT CARD RECEIVED!!!!! (card received couple weeks after approval)

If the K-1 medical exam was less than one year from the date you send in your AOS application, you will not need another medical exam.

You will require the I-693A (Vaccination Supplement) to attach to your application. If you have the Vaccination Worksheet from the K-1 medical exam, you can take it to a civil surgeon in the U.S., and have them transfer the information from the Worksheet onto the Supplement.

Make sure you call around to get prices from a few civil surgeons. We got a variety of prices and quite a price range from different doctors. We ended up paying $35.00 for the transfer of information from the Worksheet onto the Supplement.

Some civil surgeons will tell you that they can't do just the Supplement, and that they need to do the full exam again. That is not true, so just keep calling around until you find one that will just do the Supplement for you.

Edited to add: You don't need to take the I-693A.... the civil surgeons have them.

We don't have anything from the examination. The POE took everything that was in the mistery package. The only thing we have is the X-Ray. Did we make a mistake? I wouldn't think so...

Some how, I have this feeling we're going to need another examination or something. Ania has her vaccination forms from Poland. Do we take these somewhere and have them translated and viewed by an cival sergion? little lost here :help:

Should we just send in the AOS and let them hit us with an RFE?

I took Claudeth to a civil surgeon and he did the supplement and also gave her a tetanus shot, they didn't have it in Manila. He filled out the supplemental and put it in a sealed envelope for us to mail with AOS. He also made us a copy.

Hey you DO NOT HAVE to go get another physical if done for K1 at embassy within year of you filing this AOS. DO NOT WASTE your time or money, here is simple quick and very cheap solution that is for real and works.

THE form everyone is mentioning to you is a DS-3025(Vaccination Documentation Worksheet US Dept of STate) it was the form that your wife's Dr in her home country would have written on and given her and checked off a list of items and then signed and dated it. NOW the next step is to make a photocopy of that form and mail it to this address

(you will have to look up their phone number I forgot it, but do a websearch it was easy to find for Civil Surgeons)

He only charges $10 to fill out a I693A form for you, which he will do or transfer information over from that DS - 0305 physical examination form your wife had done in her home country with approved Dr by USA embassy. Then he makes copy of the I-693A that he completed to mail back to you and then he puts the original he signs inside a sealed envelope with letterhead(you do not open this, it is the I-693A supplement, you will need to mail this in with your AOS application in sealed envelope from approved US Civil Surgeon, DR L. E. ARNOLD and it is not to be opened per AOS instructions).

I would call his office first, then when you mail them your $10 check you need to write note explaining what you want and also when you call his office you will have to leave a message but they will call you back, and usually within hour or so and then you get to speak to a person and then tell them what you are doing and they will know exactly, this was great, I checked with a civil surgeon here locally where I live and he wanted $200 to fill out and sign and wanted to do another physical, when he did not need to, it was a joke, a highway robber! AND WHY no one else on here has posted to help you yet is beyond me but write this info down fast before it gets blocked or taken off here by some over zealous moderator! :whistle:

13. I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

I'm having the same problem. No DS3025 from the doctor in Madrid, and we do not know what's in the packet, and no one has a good answer to the question about a missing ds-3025. You would think to be a certified civil surgeon, they would KNOW to give this to you or at least TELL you what is in the packet before you get halfway around the world and find outsomething is missing. We have a DORA appointment tomorrow, so I will repost and let you know what happens when we get back. Weare going without it tos ee what happens. The worst possibility is that we get an RFE. :thumbs:

You don't necessarily need the DS3025. My son didn't have one and all I did was take in his vaccination records from our UK doctor. The CS then just transcribed from that instead.

A titre test at your local Doc will also see what immunisations are current if you do not have any medical records. Will need for the AOS, MMR, TD, and varicella. Also lately they have been asking for the TB skin test. The overseas medical only uses the chest xray. Personally, not everyone gets asked for the TB skin test so you might want to see if you get a RFE for that one before going to that hassle, depends on your office.

you definitely have the option of sending in your packet now an waiting for the RFE, if you get one.

If you decide to go this route, you may want to take advantage of the info about the $10.00 dollar deal while waiting, that is small price to pay for some security of having it available if needed.

Yes, have them translated into english and then transcribed by a CS onto the vacc supplement
