Hi,

i have fast question for ya ..

i been here in USA more than 2 years and for sure my medical examin... i took in my country expired. so i went to Dr. and i got my I-693. Done ! and i dont need x-ray .. thanks god. so i wanna make sure is this's the right form i need i mean I-693 or I-693A i really got confused !!! as i saw some one post here that he have had done both of them !! when i went to Dr, they asks me my I-693 Form because they dont have it there. so thank god when we had infopass, some times ago. we got the I-693 form from there. SO. when i went to Dr. i got shots there and TB Test, and was negative, thank god. so and when i backed again i got copy of my shots record with copy of my I-693 ! so my question is ! do i need to send the envelope to USCIS or just wait till my interview and give it to them ! because i dint get refuse on it but on the interview letter asks me to bring it with me ! well now i have no idea about I-693A and for what exactly. i think for the vaccination! if Yes, do u think the Dr. going to send it to USCIS from his side or should to be with me i mean I693A form !! if Yes that means i missed it !!!! or do i need to back to the Dr. ask him for it again !!! or do i need to take the form with me .. by the i dont know where is that form anybody has it can post it here i will appreciated, Hummm or the I-693 concern as full medical examinations and dont need I-693A!!!!

I don't know if you need both but you can get them both online and print them. Just Google I-693 or Supplemental I-693.

On principle, you only need I-693A for your AOS. I guess things change a little for you because the physical you went through to obtain your visa has expired and you needed the I-693 redone... so if I understood you well, you now have your I-693, but no I-693A? I would think that the I-693A matters more because CIS wants to be sure you have your immunizations straight. Anyway, I would go get your I-693A done as well.

If you didn't get an RFE for your I-693A, then simply bring it to your interview, I would say.

Like the other poster suggested, find the forms on www.uscis.gov and look over them. I don't remember what I-693 looks like, but I-693A is just a list of various diseases and boxes for the doctor to tick whether you are or are not vaccined, or whether it is or is not appropriate for you (depending on age or other factors) to get vaccined at all, etc. If I-693 does not cover that, then I-693A may be required. In any case I think it would be safer for you to get it.

ok if i understood, when i went to Dr. and i gave him my I-693 Form because he dont have it there, and when i finished from my medical examin. and i got sealing envelope, do u think he should to add I-693A Form, inside the envelope !!! or this come separate!!

I have no idea what is in your sealed envelope or if the doctor was supposed (or not supposed to) include an I-693A in the envelope.

I just looked over form I-693 and decided that it wouldn't make sense for the doctor not to include an I-693A... but I have no way of knowing for sure.

if you have the full medical done here in the US the doctor will have listed the vaccinations on your I-693.... so you do not need a I-693a... the full medical report should be in a sealed envelope and you take that to your interview....

I had my medical done here in the US and no I-693a was included....

I guess things change a little for you because the physical you went through to obtain your visa has expired and you needed the I-693 redone... so if I understood you well, you now have your I-693, but no I-693A? My prior statement was not clear.

I just looked onto the USCIS to refresh myself.

The I-693 you get overseas typically does not include the appendix A (commonly called the I-693A), which is the vaccination supplement. Whereas when you get the medical in the US, it does include the vaccination supplement.

What I should of said before is that you only need to submit the I-693A if requested. It is likely that you civil surgeon did fill out all of the forms and you might not need to submit this form.

I am stating this only as it is historically correct. We have seen time again people coming to the states and when they adjust they must either submit or re-submit the I-693 Appendix A as requested by an RFE to the applicant.

Can you call the Dr. and ask if they included the appendix A?

We requested a copy of the medical, which they gave us without a problem.

The DS3025 vaccination worksheet will be transfered by the CS onto the I-693a and placed in a sealed envelope for you to take to your interview or, if you received an RFE to submit by sending it.

The best way is to send it in with everything when you file. You should also have your own copy of the I-693a in case it gets lost, so ask the CS for your own copy if you haven't done so.

I had a full medical (I-693) done in the US when I did AOS. The civil surgeon gave me a full copy of the report that was in the sealed envelope. It included the I-693A.

It would be a good idea to ask the civil surgeon for a copy of the report they sent so you can see if they included the I-693A
