My wife came here under a K-1 visa. We have the Perm Resident Interview on 13 November. She had the required Medical Exam and Immunizations for the K-1 while in Honduras less than a year ago. My Notice of Action for the interview says to bring a completed medical examination (From I-693) and Vaccination Supplement in a sealed envelope (unless already submitted).

In addition, it is asking for the Affidavit of Support I-864 again unless already submitted.

Anyone help me with what we need to do?

If you did not get the vaccination record transfered to a supplement by a civil surgeon when you filed to adjust status, you will need to get it done before interview.

Get vaccination record transfered to a vaccination supplement by a US Civil Surgeon, and take it to the interview, you dont need a full medical done, and if the doctor wants to do a full medical, show the doctor I-485 directions showing that K-Visa holders only need the supplement done.

Thanks again. How do I locate my local US Civil Surgeon? I wonder if her doctor on base (Military Doctor) qualifies?

IN my case my wife's physical she took in china was good for one year. So if your wife's physical that she took for the K1 is not over one year old , it is still valid. In the case of the I 693 A , I went to the local Public Health department and they determined what vaccinations she needed and had them updated on the "yellow" shot record. Than I took the shot records to a local physian that was also a Civil Surgeon and the nurse transferred the vaccination records to the I 693 A form and had the doctor sign it. At the interview in Denver , they did ask for the I 693 A form and my I 864 support forms.
