My wife came over to the US under K-3 Visa, and our I-130 got approved. In Part 3 of the I-485 form, it asks:

Have you ever before applied for permanent resident status in the U.S.?

Do I answer Yes or No here?

Also, when I file for I-485 and I-765 together, do I write 1 check with the total amount for the fees for both or 2 separated checks: 1 for the I-485 and 1 for the I-765? Thanks!

If your wife has been to the USA before and had applied for a green card before then answer YES, if this it the first time for applying for a green-card then answer NO.

For AOS the FEE is $1010, this figure includes the cost of EAD (I-765) and Biometrics, so there is only one fee $1010.

Read page 7 of I-485 directions, this explains that EAD is part of AOS fee.
