Medical Exam with Civil Surgeon in U.S.

Hi,

We are in the process of finalizing the AOS paperworks and we are stuck with medical examination requirement...

***My wife had complete medical exam, xray and vaccination in Manila as what everyone must have prior to interview...MY QUESTION IS...DOES SHE STILL NEED TO SEE THE CIVIL SURGEON HERE IN THE STATES TO GET THE I-693 OR I-693A TO BE SUMBMITTED TOGETHER WITH HER AOS PAPERS.

***The only documents she has here from the medical examination is the DS-3025 which runs thru all her vaccinations...she is complete with all her vaccines except for the TD vaccine (wasn't available in the Philippines that time). Does she still need to get this vaccine prior to submitting her AOS papers.

I need a clear explanation of this medical requirement...thank you.

Yes, you need to see a US Civil Surgeon to get that form I-693A signed, and submit it with your AOS package. The CS will give it to you in a sealed envelope, all you need to do is throw the envelope in with the rest of the package.

It seems that some people bypass that by sending their package without the I-693A -- later on, CIS will require you to submit the I-693A.

It is stupid that you have to submit the I-693A since the DS-3025 is pretty much the exact same form and you already submitted that, but that's how CIS is...

And yes, I believe the TD vaccine is something you will have to get.

2005-04-14: met online

2005-09-03: met in person

2007-02-26: filed for K-1

2007-03-19: K-1 approved

2007-06-11: K-1 in hand

2007-07-03: arrived in USA

2007-07-21: got married, yay!

2007-07-28: applied for green card

2008-02-19: conditional green card in hand

2010-01-05: applied for removal of conditions

2010-06-14: 10-year green card in hand

2013-11-19: applied for US citizenship

2014-02-10: became a US citizen

2014-02-22: applied for US passport

2014-03-14: received US passport

Timeline

You need to visit a Civil surgeon in the USA to get the vaccination record transfered to a vaccination supplement. The fee for this tends to be $20-$40.

Vaccination supplement is one of the most asked questions on VJ, a little searching will get you an answer.

Our C.S. was only concerned with:

# Mumps, Measles, Rubella (MMR)

# Tetanus and diptheria toxoids

# Varicella

Thank you for the quick reply :) It is all clear now to us. I agree with you why we have to transfer the DS-3025 information to I-693A when all of the informations are just the same. Right now I am in the phone with one of the CS office. Thank you.

Make sure to bring the instruction where it says K Visas only need the vaccination supplement.

Many Civil Surgeons will insist you need a full medical- they don't seem to understand only the vaccination part is needed.

My appointment cost $120. I never left the waiting room and never even saw the Doctor. The receptionist just brought the sheet in the back for her to sign.

Shop around, the cost can vary by a large amount
