you don't have to fill out anything. Just call the NVC, speak to an operator and tell them you don't want to persue the CR1 as you have filed for AOS after K3.

If you want to update your address, do this with USCIS, NVC doesn't really need your new address.

If you want to make it more formal, you can of course write a letter to NVC to let them know you want to cancel visa. I think this is better than just letting the year pass without contacting them.
