NYC Civil surgeon for vaccination supplement -pls recommend

Can anyone recommend a civil surgeon in NYC to get the vaccination supplement? I had my full medical and additional vaccinations in January in London and now just want to get the form copied over to the one we need for AOS.

I know I can call USCIS and get the list but if anyone could just let me know where they went that would be great!

We found one that was walking distance from us (Bensonhurst/Brooklyn)

I came, fill some paper.They gave me shots, and told me to come back for three days to check for tuberculosis test, but I couldn't come back that day, so they told me just to call and let them know how it looks like.

I payed $200 for I-693 and vaccination supplement. They were the cheapest that i found since all other doctors that i called from my zip were more expensive and had different price.

I think we paid $35 for the supplement or something. It was cheap.
