We are about to submit our AOS + EAD forms, but seem to have lost the vaccination certificate. We are applying to get a new copy which will take a couple of weeks. Should we submit without this or should we wait until we have it? Thanks for the advice.....

You mean a vaccination supplement you get from a civil surgeon in the US?

I mean the vaccination certificate issued by the medical examiner in London in preparation for the interview at the Consulate. We could get an RFE as suggested, OR could we send it in separately once it arrives - once we have a case number assigned and before an RFE is issued?

Hopefully, someone will correct me if I am wrong. But as far as my understanding goes, you still need to take your vaccination supplement to a US Civil Surgeon, so he can give you a I-693A to send with your AOS package.

Hopefully, someone will correct me if I am wrong. But as far as my understanding goes, you still need to take your vaccination supplement to a US Civil Surgeon, so he can give you a I-693A to send with your AOS package.

Almost correct :D

You have to take the vaccination worksheet (the one from the foreign medical) to a USCIS approved doctor in the US and have the vaccination supplement filled out. And the vaccination supplement IS the I-693A :)

We went to a civil surgeon to get the supplemental and didn't have any problems. I guess you could try to do it without the supplemental but there is always a possibility of receiving an RFE for it. Good luck

Yup. But some people don't have one. In this case I would take all your vaccination documents to the civil surgeon.

Wow! This has been extremely helpful. I really didn't understand what the I693 was or how it was to be created and you guys have helped clarify that for me - and probably saved me a bunch of time and RFEs too!! Thanks!!! We'll be looking for our local US Civil Surgeon and paying them a visit soon.

Careful, don't forget the letter "A" behind it.....the I-693 is the "complete medical".....the I-693A is the vaccination supplement. :)

I believe, not 100%, that our doctor used the I-693 without the "a" and just wrote all my vaccination history in the box that said "history transferred from other medical form" or something (dont quote me on the title box). I figured they knew what they were doing, is this correct?
