My wife has her medical exam on Thursday in Kyiv, Ukraine. Does anyone have some good links describing exactly what to expect? In particular, I'm wondering if there are any forms she needs to fill out and whether or not I can help her with this before she goes to her exam. We will be adjusting status once she comes to the USA on K3...what forms/documents can we get from this medical exam that we can use here in the USA? She has a document from her local medical facility showing she has all of her vaccinations (it has their official seal). I know there are some forms that these can be copied over onto but not sure if she can do this at this medical exam or not. Anyway, I would greatly appreciate a crash course in this!

From what I remember my wife had 4 forms, the doctor will give her these, DS-XXXX I can't remember the actual form numbers, but all they ask for is name, DOB, address, etc. She'll give them to the doctor who does the medical exam and fills them out. They'll draw blood, take some x-rays, etc. All in all takes about an hour and you'll get the results in a sealed envelope in one week. Don't open the envelope. Nothing to stress about. I've heard some doctors will transfer the info from that medical exam to the AOS one once she's here. My wife's didn't and charged us 300$. In either case she'll want to take her vaccination records here so she won't have to get them all over again.

Also, she just told me that her vaccination records from her local clinic are in Ukrainian...does she need to translate this prior to her medical exam?

Her vaccination records will be transcribed onto Form DS 2053 by the clinic (no translation of vaccination document required). They should provide a copy of this form to her to use for AOS, so make sure when she picks up her results she looks for this form.

A little more. First don't forget to send money. My wife took extra above the one hundred for the exam. Actually one exam was $100 and one was $75 (US Currency) and I don't remember which was which. One was for my son and the other for my wife. Anyway she was short a few shots and needed an extra $20 to cover the shots.

Also she arrived early (8am). She was finished by noon and went back to pick up the results at 3. And this was all a few weeks ago. No advanced paperwork. No studying except for the blood test (just kidding). The questions were routine. Asked about all past medial history, operations, tattoos and the history of any scars. It was all routine, they were polite, professional and quick.
