my father's name is say alpa beta gama.

1.gama is the surame but the form asks for the first name also..will it be alpha beta or just alpha?

2.in the mother's name section,it says miother's name(maiden name) ..does it mean that i have to write the current surname and then maiden name in bracket or does it mean they want just the maiden details of my mum.

3.does it make any difference if i dont know the palce of birth of my parents

please help..Thanks in advance

1. You mean first name, middle name last? Surname is last name.

2. I just put my mother's maiden name

3. It shoudln't, just put unknown

You don't have to, just write the first name last name.

Example: Robert James Smith.

Smith is the family name or Surname. Robert is the first name and James is the middle name.

In your own example Alpha is the first name, Beta the middle name and Gama the last or surname.

The G 325a doesn't have a place for the middle name, so it is not written.

Your mother's maiden name is used so Betty Jane Jones. Smith is her married name but you don't indicate that. They actually don't care what other names she ever had. It's her birth name that matters.
