Get it from GMP. The form can be downloaded here http://www.gmp.police.uk/mainsite/pages/subjectaccess.htm

Someone correct me if I am wrong but I am quite sure that you want a national Police Report, the police call it a Subject Access Request. The Embassy in London want to know if the Police National Computer holds any information on you. As the PNC is a nationwide database it will therefore cover all areas of the UK and not just Manchester. In my force, Thames Valley Police, we wouldn't process a request for a Police Report unless you came into the station itself with ID because any Tom, ####### or Harry could download a form and then send it in. Therefore, when you go into your local nick make sure they give you the form for a national Police Report and not just the local one.

You DO want the subject access form, however, most regional police forces have their own version of it on their websites. If you can find it online it is perfectly acceptable to use this and send it off through the post. With this application you will still need to send some proof of ID. This can be passport (not recommended) a utility bill, or an NHS card. I sent in my NHS card and got it back within 2 weeks along with my receipt.

You DO apply to the local police force where you are living at that point in time, but then your local constabulary forwards it to Scotland Yard where it is checked on the police national computer. So, all police reports actually do go through the Met Police, although you only apply to them directly if you live within their boundaries.

Remember to allow a good 8/9 weeks for processing. The reports are supposed to be with you within 40 days, but that is 40 days from the date of their receipt...and the receipt often takes up to 3 weeks to be issued on its own.
