I noticed on our form near the bottom where it says results that the doctor put a check mark on the first box which says; Applicant may be eligible for blanket waiver (s) as indicated above. Then the appropriate boxes above are checked off where it says not appropriate to age.

Is this ok? Has anyone turned in their form filled out like with without any problems?

At least it isn't checked off saying applicant does not meet immunization requirements.

Those results are perfectly fine. Many people have submitted the supplement with a "blanket waiver" checked off for vaccinations that are not age appropriate.

A civil surgeon fills this form and signs off on the form, not the petitioner. The form is then placed in a sealed envelope by the civil surgeon.

If you are adjusting from K-Visa and it has been less than a year since the Visa Medical, all you need to have filled out by the US Civil Surgeon is the Vaccination Supplement, (I-693A)

Thats what exactly appeared in my vaccination record given by St. Lukes and i didnt have a problem with it. Good luck
