HELP - VACCINATION RECORDS?

I am about to submit my wife's AOS application and have realized that they require a copy of my wife's vaccination record. The only form she has in the DS-3025. Is it sufficient to submit that or do we need to go to a civil surgeon and get a I-693 form??

It all depends if she got all of her shots. My wife didn't get all of hers as they didn't have the TD shot to give and she needed her second dose of varicella. I also had her get a TB skin test since some people had been requested to do so here in the U.S. If you get anything done here in the states you will need to have her original medical and the new stuff transferred over to the famous, Appendix A Supplement to Form I-693.

There are others who only turned in their DS-3025 medical form and nothing was said at all.

You just never know.

Thanks. She has had all her shots so i think im just going to submit the DS 3025 form and hope for the best. I guess if they require the 694 they will request it from me later..

Why risk delay? Most people DO get an RFE requesting the I-693A Vaccination Supplement. If not that, then they are requested to bring it to the interview. (our interview letters states: "YOU MUST BRING THE FOLLOWING ITEMS WITH YOU: A completed medical examination (Form I-693) and vaccination supplement in a sealed envelope, unless already submitted") It is easy and fast (and usually not that expensive) to get it done at a civil surgeon.
