Appendix A Supplement Form to I-693

I thought I had this drill down after reading a million posts about this form for the last couple of months but apparently not. As the nurse was finishing filling out this form she says, you know there is another part to this, say what?????? I haven't found one post on VJ that even comes close to saying there is another part to this form. Maybe I should have but I didn't even want to ask to see the form she was talking about. We both just made a joke about the whole process and away we went.

So my question to all, is anyone aware that there is another part to Appendix A Supplement Form to I-693 also affectionately known as I-693A? It's a single page form correct?

I really didn't want to get into it with her about it but she said something about blood test. I told her the usually stuff, MJ had her physical in July 2007 and the instructions on the I-485 for AOS clearly states that form blah, blah, blah can be used.

So we got them to finish the form off and seal it but I just had to post this to find out if something changed over night.

The good part was that they didn't charge us for filling out the form but MJ had to finish off some shots.

  • varicella 2nd shot-$100
  • TD shot, $40, wasn't available in Manila
  • TB skin test,$20 got this done just to make sure.

the nurse must be talking in general. In deed the I-693 A is the "supplement". But K1s as you read only need the supplement for the AOS.

07-10-09 Biometrics appointment

20-10-09 Second biometric (29 was the date)

I think most everyone if not everyone who has filed the I-485 is well aware of the "vacination supplement". It does get discussed quite a bit.....
