Hello People!

WellI have now completed my forms for AOS my query related to the medical I had in jan this year - I know i have to include evidence of my shots and i do have a copy of what they did at Knightsbridge - is this all they want me to include? just copy the paperwork and include with pack? for some reason i feel there should be another form but cannot find it if there is?

All help and advice greatefully received

Most people have the shot record received at the medical transcribed by a US civil surgeon onto a form that is commonly referred to as a I-693a. This maybe what you are thinking of

six of one half dozen of the other.....

Give it a go ... some get a RFE for the supplement in which case you can have the vacc worksheet transcribed onto the I-693A but then some dont and the worksheet is enough.

It all depends on who is looking at your case. Oh and if you dont have all your shots you will probably get a RFE.

My experience, I submitted worksheet, fully vaccinated, still got a RFE for the supplement at interview. $10 to Dr Arnold in Texas to have it transcribed and bob's your uncle.

I took Claudeth to a civil surgeon to do the supplemental, they are the ones authorized to do so. They will give you a sealed envelope with the I-693A. They should also give you a copy. Claudeth have to have a tetanus shot because Manila didn't carry that vaccination.

If I remember well from AOS forms and instructions... copy of your vaccination sheet given to you at the time of medical is enough and it specifically says you don't need the I-693a. That's for AOS after K-1.

That's all I sent with my AOS appl and I got no RFE for that. My RFE is about tax info so I guess they accepted my vacc sheet from medical done 6 months ago.

24 July '07 - AOS application received by Chicago Office

09 August '07 - Biometrics appt letter

24 August '07 - RFE for I-485 (incomplete tax info sent!)

28 August '07 - NOA letters for AOS, EAD and AP

29 August '07 - Biometrics

AP - 12 October '07 - hard copy: multiple travel, valid 12 months

EAD - 18 October '07 - hard copy: valid from the date of 1st notice of card production

SSN - 30 October '07 (married name), applied Oct 18, same day I got my EAD

DL - 11 December '07 - driving test at DMV San Francisco. Yay! Now I can drive on the "wrong" side of the road!

21 November '07 - AOS Interview (Letter rec'd Oct 15) ----- A P P R O V E D ----- A P P R O V E D ----- A P P R O V E D -----

07 December '07 - GREENCARD IN MAIL

Peace and quiet... apply for removal of conditions - August 2009!

Not quite correct. You dont need the I-693 (full medical exam) if one is completed within 12 months before submission of the AOS application. The I-693A is the supplement form to the I-693 and should be completed by a civil surgeon. Alot of countries do not complete the list of required vaccinations during the consulate process. For instance, alot of filipina's do not have the TD vaccination and are required to do so at AOS stage therefore requiring the vaccination supplement to be filled by a civil surgeon. Also note that the TB skin test is something new that has been happening recently as well here in the US whereas we where only required to have the chest xray for consular processing.

Again not everyone gets asked to have one done.

Many thanls again ...I keep seeing Dr Arnolds name mentioned - i dont live anywhere near texas - does he do this via postal system? I am presuming not as in some cases it may be required to adminster further test?

He does do it via post... If you need updates or boosters that is another thing

Check this thread, If you need extra vaccinations I would think that you would be able to have them anywhere but provide Dr Arnold with proof that it is done. The girls in his office are great at answering questions.
