What exactly is inside the medical anyways?

After following Misa's advice and requesting a copy of my medical, I finally received a call back from the doc, who had left a message at my house. They requested that I email them and tell them exactly what I need.

I don't know exactly what comes in the medical package, so I wasn't really sure what to request. I basically asked for everything but the x-ray (just said I would like a copy for my own records, so the x-ray wasn't important.)

Does anyone who has had a copy for themselves know if there is just a certain section that the doctor that examined you fills out? Just not sure what to say if she emails back again asking for specifics of what I need. I would like anything that the doctor wrote just so I can have the reassurance that everything is good when I go for the interview.

The medical contains these forms (the Canadian version, don't know why some physcians don't use the official Department of State Forms). There are several spots where the physician checks things off and can make notes:

- DS-2053

and worksheets

- DS-3024 (chest xray worksheet)

- DS-3025 (vaccination worksheet)

- DS-3026 (medical history worksheet)

and lab results for the chest x-ray, HIV test and syphilis test

There's room for the doctor's comments on the DS-2053 and the DS-3026.

They should just give you the entire thing if you're paying for it (I would hope).

I tried to get a copy of my medical and the answer was.... "it is property of the US government: so no copies for me. Hmmmm.... if it's their property, why do I pay for it ?

I was told that I could request a copy for $50 ... and also that they would not send me the medical if eveything was not A-Okay ... I had received the medical so I was not concerned

That's pretty ridiculous especially if you were willing to pay for a copy. I know that some doctors don't keep copies on file though.

Can I say how much I enjoyed going to Dr. Seiden's? Their laidback attitude = i.e., didn't really care about stuff was awesome.

The Dr in Calgary said we could not have a copy for ourselves either. he said the AOS medical was completely different. grrr..

wow Misa, the copy of the medical documents the doctor's use are awesome. Where do you find these things?! haha

Adding my two cents here...

I had my medical completed by Dr. Al Laity in London, ON. His staff and himself were very efficient. He took about 5 minutes to ask me some questions and evaluate me. I did answer yes to one of the questions, and he told me it was no big deal and said he would leave it off.

Anyways, my paranoia kicked in later that day, and I called back to ask for a copy of the medical. The receptionist said no problem, and that she would send it with my sealed medical by fedex which I had paid extra for. No extra charge for the duplicate copy of medical though.

I'll be picking up my package today.

All in all, great service, fast and friendly. X-ray technician is in the same building, but takes 1 hr to get x-ray.

Recommendations as to which one is better and why...

That's exactly what I was told, Len. And I replied with the very same question.... "How is it their property if I'm the one who is paying for it?" Got me nowhere but at least I asked. lol

In any event, the officer at the POE gave me a copy of all the medical information when I crossed.... so to Medisys in Toronto.

The best thing to do I would think is call ahead and ask them if you can have a copy of your medical made up to use for AOS. I have read quite a few reviews on Seiden, and can't remember a complaint so far. I haven't read as many reviews about Medisys, but I don't recall any complaints about that doc either, off the top of my head.
