Hi everyone,

I am preparing I-134 for my wife and I have few questions:

Question 3:

Do I put her maiden name or name after marriage? she changed her name and had the new passport issued after marriage. petition was also filed using her new name

Question 10:

Do I put her name here and date i-129f was submitted?

Question 11:

I am not sure what to put here.

In my case, there were two names, wife and daughter. I submitted petitions previous to filling out the I-134.

Question 10:

Actually, I put her name here and the date I-130 was submitted.

yes, put her name for question 10.
