In reviewing the VJ.com website, it seems my wife (here on a K1) will need to fill out Form I-693a as part of her AOS package. Where can I find this form???? Did you all do this???

When you have the vaccination record transferred by a local civil surgeon, s/he will provide the form. It HAS to be filled out by a CS.

The I-693A is 'the' medical/vaccination supplement - this is used when filing AOS. Niether the beneficiary, nor USC, completes this, it is done by a civil surgeon. The civil surgeon has these forms on hand and will complete it for you - they may (most do) provide you with a copy of what they complete and give you a sealed envelope to send in with your AOS.

In most cases this is all the medical documentation needed to file (with your AOS) if your initial medical was performed less than one-year ago.

The civil surgeon has these forms on hand and will complete it for you - they may (most do) provide you with a copy of what they complete and give you a sealed envelope to send in with your AOS.

In most cases this is all the medical documentation needed to file (with your AOS) if your initial medical was performed less than one-year ago.

OK, now I'm very confused. I'm preparing my AOS paperwork for my wife. Who is this civil surgeon person??? Where can I find this person??? Is this something I need to do before I send in my $1010 and all forms??? HELP!!!!!

FEDERAL REGISTER INTERIM REGULATIONS at USCIS website, I quote:

"C. Medical Examinations

According to 8 CFR 245.5, aliens seeking to adjust status are required to undergo a medical examination performed by a designated civil surgeon to determine whether they are inadmissible under section 212(a)(1)(A) of the Act. To date, applicants for K nonimmigrant visas have been required to obtain a medical examination abroad pursuant to Department of State regulations at 22 CFR 41.81 prior to entry, and the medical examination is not repeated if they apply for adjustment of status within 1 year of the dat e the examination was performed. They are, however, required to submit with the adjustment of status application a vaccination assessment completed by a designated civil surgeon in order to establish their compliance with the vaccination requirements under section 212(a)(1)(A)(ii) of the Act.

The Service will continue this same policy for the K-3/K-4 nonimmigrants. K-3/K-4 nonimmigrants who file their adjustment of status application within 1 year from the date of the medical examination overseas will not have to submit an additional medical examination. However, the Service notes that applicants whose medical examinations overseas revealed a “Class A” or “Class B” condition (as defined by 42 CFR 34.2( B ) ) must establish upon application for adjustment of status that they complied with those cond itions imposed on the initial admission. Failure to comply with those conditions means that a new medical examination will be required.

OOps and here's what people "call" form I-693a which is actually Supplemental I-693.

As someone else posted the DCS normally have their own form.

And yes it's best to submit the form from the DCS with your application. Try to get a copy tho [DCS will give it in a sealed envelope for you to submit] as USCIS for some reason "misplaces" them once in a while.

Yes you need to do this before you file the AOS.

Read the K1 AOS guide before you do anything.

What I do not understand is where the Civil Surgeon gets the information to fill out the I-693A. My fiancee (wife now) was not given any copy of DS3025 at her medical in Moscow. She has no documents from this medical. She submitted her sealed envelope at the point of entry - that's it. She has her vaccination record in Russian. So, what does Civil Surgeon do with this? Or we have to translate this record into English?
