Im filling up my DS form, I have lots of Qs here help me pls.......

1. the DS form that the embassy sent me is expired, is it ok to use or download online?

2. In DS156 the embassy sent me it has no barcode, will I fill up online instead?how to get the barcode if necessary?

3.On Q#8 it says: other surnames used(maiden, religious, professional, aliases)------- Im annulled, will I put my ex surname on here?

4. On Q#9: other first and middle name used--------will I put my maiden name here?

5.on Q#17 there is no space for annulled, what will I choose?

6. on Q#22 when do you intend to arrive in the US?---------we have no definite day yet cause we don't know if when will they issue my visa

1. as for me i wil use the downloaded form so i can just have my forms printed with my informations. (the expiration on my forms are 9/30/2007. but my interview is on OCT 4)

2. there will be a barcode on the downloaded form (i think, coz i havent filled mine as well lol)

3. if it is not applicable, just put N/A. your already annuled so i guess you will use your original surname

4. ....

5. yes i would like to know this as well lol :D coz i am annulled too! i guess ill just check the divorced

You can fill up your DS156 Application online

This is from the USEmbassy is Manila site:

TRANSITION TO THE ELECTRONIC VISA APPLICATION FORM (EVAF)

Starting 01 November 2006, ALL nonimmigrant visa applicants must use the online Electronic Visa Application Form (EVAF) or DS-156 Form. The EVAF is free of charge, will speed up the processing of visas, and reduce waiting time. It is also easy to use because it is the same application form that is currently used, except it is online.

Handwritten or typed visa applications will no longer be accepted by the Nonimmigrant Visa Unit in Manila. If applicants do not have an EVAF on the day of their scheduled interview, they will have to reschedule their interview through the Call Center or through the VisaPoint™.

For more information on EVAF, please visit the Nonimmigrant Visa Applications Forms page http://evisaforms.state.gov/

After you fill it up you send it and they will send your file back and you can print it out.

weird, huh?!?!?!?

anyway, thats what im trying to point out in my reply (see post #11) in this POST. it seems that USCIS is not well coordinated with regards to documentation, inspite the pressure they're putting in this kind of application.

i just hope that the new price scheme will make some improvement with regards to documents.

Lastly, I observed that the DSFOrms included in packet 4, may not have a bar code but has a control number . It is indicated above the expiration date.

Example: Approved OMB 1405-0133.... I guess it is.. :unsure:

hi alexandra.....glad to know that someone is sharing my confusion:).....also, what about the form validated by the BPI?.the form is provided by the bank and the only option we can do is to fill it out.that wouldn't comply anymore with the requirement that it should be filled out online.......can anyone help me what instruction to follow?

I read on this forum that it can be hand written neatly, no need to use typewriter but you can do so ;)

fill out(handwritten) the form given by the bank and do the second copy online.

i saw from previous thread that an applicant, filled-up the forms online & just attached the form bank validated form..if you're just really not in the mood of doing it manually.. :lol:

I dont receive my pocket 4 until now it makes me confuse what i really need to do? do it online or get a request to SLEC so that USEM can mail it during my medical exam? and I heard from a neighbor and someone who is in US now to make sure i have a barcode and only online is the only way i can have it?

request a pick up to SLEC and you can get it after 3 days, you need to download the DS156 online to get the bar code, for the DS156K and DS 157 you can use whats on packet 4, thats from my own understanding in asking the forums several times :P

you can call either SLEC or USE for the request, my friend call SLEC cause its cheaper and she pick it up yesterday....either ways....

ELECTRONIC NONIMMIGRANT VISA APPLICATION FORM (DS-156) http://manila.usembassy.gov/wwwhnv15.html

Items 1, 3-5 must contain the applicant’s passport number, issuing country, and issuance and expiration dates, as shown exactly in the applicant’s passport. (For applicants whose passports were issued in Manila, please leave “Country” and “State/Province” fields blank in Item 2.)

Item 6 must contain the surname(s) of the applicant as written on the applicant’s passport.

Item 7 must contain the first name(s) AND middle name of the applicant as written on the applicant’s passport.

Items 8-9 are only for other names or aliases of the applicant.

Item 15 should include the address of the applicant’s permanent local address.

Female applicants with legally terminated or annulled marriage must tick Single (Never Married) on Item 17. However, they must explain to the interviewing officer the history of their previous marriage.

Item 20 should include the name of the applicant’s current company/institution, and not the name of the applicant’s supervisor, unless the applicant is self-employed or works as a personal employee. For business owners, please specify registered business name.

Items 24-25 require the complete information where the applicant will stay in the United States.

Items 30-31 must contain information about each U.S. visa issued and each visa refused, including tourist and other types of visas.

Item 37 must contain the status of the applicant’s relative(s) in the United States, i.e. legal permanent resident or American citizen.

Item 38 must be completed personally by the applicant.

Anyone who helps an applicant to complete a visa application must also sign the application and include his/her name, address and relationship to the applicant in Item 40.

Item 41 must bear the applicant’s signature and the date the applicant signed the form.
