Hey Guys,

Just trying to fill in the form DS 156 and i have some confusion here esp when i typed the cellphone numbers on spaces provided it gave errors... so tried to delete the + signs for +63( Philippines) and + 1 ( USA) is this okay??? Another question it asked who will pay for my trip.. ? My fiance will pay for it .. it is okay?? last question...it ask who will travel with u.. if the visa will be granted we plan that i will be traveling with my fiance .. shd i put in that information?? Please help.. and thank u so much...

Hi there. Yes, I also deleted the plus sign (+) for the phone numbers. I also wrote my fiance's name since he'll be paying for the trip as well as placed his name coz he'll fetch me here and we'll be travelling togeter.

thank you very much for the quick reply... by the way I dont have telephone number of my previous employer anymore since i worked there 10 yrs ago .. and also telephone number of my skol.. what shd i do?

hey guys! i just wanna know if its ok to leave question no. 22 when do you intend to arrive in the US blank or i really need to put tentative date? because i read alot of posts here not to leave any blank in every question. im waiting for you replies so i can finalize my ds156 form now. thank you so much in advance....

oh i have another question to ask but its about ds157 on no. 17 about the dates of attendance, do i have to put 2 dates one is for when i started the course and second is when i finished the course but im having problems coz it wont allow me to enter 2 dates, what should i do? pls i need some help. thank you so much.

i just put "when visa is granted" i just followed the example form
