Where can we get I - 693A form from?We are going to visit some surgeons tomorrow and we read that it would be better to bring it with us to the doctor.

One more question - the embassy doctor said that they do not do usually VAricella shots in Bulgaria and on the vacination sheet it says "not regularly available".Do you think I will have to take a shot?Has any of you guys experinced that?

The civil surgeons have that form on file. They will fill it out for you and should provide you with a copy along with a sealed copy to submit to submit with your AOS paperwork.

If you an provide a verbal (or written) record of having had chicken pox in the past, then you don't require a varicella shot.
